Making Appointments: Appointments are of two types: Complete Exams and intake visits may take 20-30 minutes, so please allow ample time. Treatment or follow-up examinations require 10-20 minutes. We try to stay on schedule, although emergencies sometimes arise. If we are seriously delayed, we will notify you beforehand. Please remain clear and concise in your health discussion by sticking to chief complaints. We may call patients in the waiting room out of turn if urgency was conveyed or for a postoperative exam. We ask for your patience if you have to wait; we know your time is valuable and will do our best to stay on schedule. Please notify the front desk upon arrival.
Your Appointment: On your first visit, we will create a permanent personal record. Please arrive 15 minutes early for your first visit to complete a brief medical history and a registration form. You may print these out at home and bring completed paperwork to your appointment to save time! Kindly have your insurance information with you along with any current medicines, even if they are out of date. If you have been under the care of another doctor we may ask your permission to transfer your records to our care.
Your insurance card and photo ID are required at the time of EACH appointment.
Read and understand your insurance policy. You policy is a contract between you and the insurance carrier. Read it, understand it, and ask questions. Your insurance does not automatically cover everything. Even different policies from the same insurance company can have different requirements. It is your responsibility to know what your policy covers and what it does not, and also, whether you need referrals or a primary care physician listed.
For thoe patients without insurance, payment is required in full at the time of service. Upon request, we will be happy to provide you with an estimate of the cost for specific services before your appointment. We accept cash and major credit card companies.
Co-payment is to be paid at the time of service.
If you cannot attend a scheduled appointment, you need to call at least twenty-four hours in advance to notify our office. Patients who fail to inform the office of the above or fail to show for a scheduled appointment, will be charged a $40 fee. This charge will be collected at the time of your next schedule appointment and/or will be billed to you directly. We will not bill your insurance company for missed appointments.
If you are more than twenty minutes late to a scheduled appointment without a phone call to alert us, you will be charged a $40 tardy fine. This charge will be collected a the time of your next appointment and/or will be billed to you directly. We will not bill your insurance company for fines.
Under special circumstances, payment arrangements may be made with our billing department. Payments must be paid on a monthly basis. Payment arrangements apply to the existing balance only.
